• Co-Ed Divisions Men must be 30 years old or older. Women must be 18 or older.
• Men's & Women's Open players must be 18 years or older.
• Team's must complete a Team Application Form and fax to Steve Green on (865) 403-5802 for each season.
• Player's already registered, wishing to to have dual registration to be able to play in the Men's or Women's Open Division, must complete the Dual Registration Form, and submit with team rosters.
• You will need to photocopy your driver's license as proof of age and submit the copy with the appropriate forms.
• Each new player must fill out an Amateur Player Registration Form.
• To drop a player from your roster you need the Add/Drop Form.
• If you wish to transfer to another team, you will need the transfer form
• Mail or give the completed forms to the appropriate team rep, along with a check to cover registration fees. Fees vary from season to season, depending on the number of games and the number of people on the team.
• If you wish to start a new team, or for every new season, the person in charge of the team will also need the team roster. Contact Steve Green for more information about starting a new team (865-577-6569).
• Team AAW waiver form (for Fall Season Only).

Save the forms you need to your PC then print them. They are in MS Word 95 format, so no matter what word processor you have, it should be able to read them.
